I would rule that, if defeated in a contest then that character
could not keep on fighting until he/she had recieved medical aid.
If they were subsoquently attacked, then any result would bump up
their hurt level.
I think framing of intentions is important here. People don't always
fight with the express intention of killing their opponent.
