Well what did you expect! There is a reason that Orlanthi are such a popular character type you know!

ISTR there was one initiate, but I may be mistaken - very few examples, but then there are very few examples all told...

This is certainly the case for Characters starting as Initiates, I'd need to check the rulebook (chapter on Theistic magic) to be certain for characters initiated "in play". If you wanted to start out with an initiation ceremony I would probably treat this as "starting as an initate" in terms of what the character looked like.

You have to improvise all uses of the Affinity, where devotees can use known feats without penalty and also improvise "new" feats

I'm guessing that an Initiate could spend HP to learn specific feats, in the same way that a Devotee can learn new feats (I'm sure someone will correct me long before I get home to check if i'm wrong!)

I've been thinking about feats and affinities and am considering a modification to the relationship between the two - Per the rules a character has a rating in an affinity and knows all feats at that rating - When a new feat is added, therefore it is automatically at this level. (example without recourse to rulebooks - please forgive if my faulty memory makes a hash of it...- An Issaries trader has a "Communication" affinity of 15W. He is travelling to Dagori Inkarth, so decides to re-enact "Issaries Trades with Argan Argar" and so develop the new feat "Deal with Trolls" which he gets at 15W - his apprentice (also a devotee, but with a score of 17) also wishes to learn the feat - he gets it at a score of just 17...)

Furthermore this way of assigning values to feats means that all devotees of that God are equally adapt at all facets of a particular affinity (All Vangathi are as good at "Fly Fast" as they are at "Aerobatics")

In order to provide more diversity, without a major change, I was considering giving Devotees a seperate rating in each feat, with the Affinity rating been equivalent to the lowest rating. This might make increasing an affinity more expensive, though, and might give problems with "in game" promotions from initiate to devotee...
