I would imagine that the devotees are the ones who do all the detail ritual stuff (not to mention practice and prepare ahead of time), while the initiates provide the congregational response.

I supect a properly maintained shrine or temple would effectively act as a favorable conditions modifier, making it easier to do the ceremony right and for it to be duly noted by the universe, but I would imagine that Sartar could have led the celebration of Orlanth's holiest day from a mud puddle and made it work.

I would imagine that certain specialists do travel to major cult gatherings, but no doubt it has a slight but distinct distancing effect on their relations with the rest of the clan ("Well sure Branbig is a solid man, its just that, you know, what with going off to town every year to celebrate Gustbran's holy day he misses the ceremony to the fertility spirits of our field, and you know, how effective are his plow blades when he isn't in touch with the field spirits?" or similar minor quibbles)
