I've always used it as a provocative, perjorative term that Heortlings use, not just the standard word used in "polite" speech. In other words, I think heortlings have a word for sorcerers in the same way English has the word "caucasian" and Meldek is the equivalent of "honky," "cracker" or "redneck."

I belive it first appeared in print in the Arlaten the Magus section of Strangers in Prax, when members of the New Pavis crowd yell it at Arlaten prior to trying to pogrom his butt. If that's what you meant by "originate."
