In my campaign I've just had two changes in the Clan Inner ring. The
first was due to the Chief scheming to get the Ernaldan Priestess
booted off as she had shown him disloyalty (rightly so as he was a
traitor as it turned out). He engineered a vote of the Inner council
and she was summillary removed. He then had a stooge nominate the
Priestess he wanted instead and ensured he got the votes from the Inner
ring. There was a degree of intimidation involved as the Chief had a
large band of cronies he could rely on to fix the vote.
The second occurance happened on the traitor Chiefs death (he'd been
consorting with chaos the naughty boy). The Outer ring voted on
candidates who were nominated and seconded (after they gave speeches
why they should be elected) and they were then voted on by the full
Outer ring. The new Chief was then elected from the Inner Clan ring.
Hopefully all that made sense and wasn't way off.
