The Underworld is as far away as the sky. The Underworld is not the under earth. There is the surface earth, which includes caves and mines where humans go. The deep earth is visited by dwarves and trolls.

On the other hand, there are some entrances to the Underworld that can be found in the middle world, including the HellCrack in Pent, the sink of the Syphon River near Heortland and Magasta's Pool. And some of the most holy Uz shrines in their deepest complexes hide entrances to the Underworld. These entrances are not the easy route to the Underworld, typical journey times are weeks of falling.

The quickest and easiest way to the Underworld is to die. Almost all humans, on death, go to the Underworld on their way to their next stop. The big issue with this route is that it is difficult (but not impossible) to get back.

Many cults have myths and or ceremonies that allow worshippers to enter the Underworld.

During the Greater Darkness all worlds partially or completely merged with the Underworld, so pretty much any geography can be found there. Most notably, the Underworld is the otherworld of the Uz and also of the Aldryami. The Aldryami areas of the Underworld are as inimical to humans as the Uz areas but I cannot imagine the Aldryami areas being dark. And each night, the sun passes through some areas of the Underworld.

Most of the Uz were driven out when the Sun entered their home. In some areas of the Underworld, the light never penetrated and the greatest spirits of the Uz still live there. But the Uz known on the surface world are like just humans, at once not strong enough and also too free willed to go and live with their deities.
