    I'm not a gynaecologist, but part of my job does involve fertility treatments for childless couples... Your theory is plausible biologically, but runs up against the problem that until Naxili Garang mucked things up, trollkin were born singly. OK, maybe they were all superior trollkin in those days, but you still have the problem of some single births having been trollkin and others dark trolls.

    Daniel McCluskey's theory about Black Eater invading the womb is a fine theory for those who want it all to sound mythic rather than RW biological. Makes sense of the idea of needing purification after a trollkin birth, too. Even if it's not literally true, it may well be what the XU midwives believe - - - which is really all that matters in-game.

    On the related subject of trolls using ultrasound to see if people are pregnant, remember that modern equipment can't detect pregnancy (and whether or not it's multiple) until the seventh week in humans. And I see no reason to assume that uzko darksense is as good as 20th century ultrasound gear, although the Mistress Race may be different. More likely you'd have to wait until at least the start of second trimester, and as somebody else pointed out, maybe it's just too risky for the mother to abort at that point. Note that troll gestation is longer than ours: 45 weeks instead of 40.

     I'd also add that AFAIK (and I'm not personally involved with side of things) even modern ultrasound equipment only produces an image when the equipment is pressed against the mother. So to sense much you probably have to stick your snout up pretty close. Which may be fine for an XU midwife, but probably is considered a bit antisocial (or over-friendly) coming from anyone else...
