Let's see I suspect in part that it's number 3 partially because most of their big combats have included an NPC, the clan champion, who goes through the opposition like a buzz saw. At least a quarter of the sessions haven't had any dice rolling never mind fights.

Agreed. I'm as surprised as David. It makes me think that your game has either:

Lots of healers hanging around in combat really lucky die rollers
bad guys consistently weaker than the Heroes

Several times in my game, the whole Rass Game party has only survived by burning every HP they had available. Like when the 5W2 trickster with a knife attacked 3 heroes with 8 total followers whose skills were 1W tops. He nearly killed 'em all and would have but for HP.
