I think , in practice, I probably would too, but it's something easliy "tweakable" by an individual GM to define what level he considers to be above that achievable by normal means

Of course! - Or the same quest with more stringent requirements / powerful foes

It is something in which the PC's could have an input (If you want to improve your skills above "X" you should suggest a myth/story which you are following to do so) - The GM would still need to actually work out the fine details, but would have something to work from.

Also you needn't necessarily enforce "A Quest for a Skill" at the point that you wish to improve the skill over the threshold. Merely in order to pass the threshold you must have taken part in a suitable quest in the past - so maybe the regular Sacred Time worship is enough to go from 20 to 1W in any skill...
