Followers keep pace with their leader to some extent, one option would be for a surviving follower[1] to step into the leadership role with the player only now filling in all the full details of the character. Obviously the new character would be slightly off the pace (primary ability 8 lower than for the deceased hero) but in a well advanced campaign that would still be a whole lot better than a starting character.

Some social connections and alliances may even be transferred undiminished to the new character - plus any other surviving followers, inherited magical items etc.

[1] Obviously having a surviving follower at all might require a bit of narrator intervention as followers usually fare worse than their leaders.
