Normal, spear-carrier followers have a single keyword at 17 (or some
other suitable level). This never changes. The player cannot change
it and it mentions that if you want a follower who will improve, to
get a sidekick instead.
Retainers don't get better. However, their loyalty can all be managed
through a single relationship (which helps conserve those precious HP)
