How about where the hero wants to protect someone use the consequences rule for hero and followers.
Only the hero takes the position of the follower in the consequences and takes the higher level of wounds.

So in the fight or rescue the hero acts normally with support from the one being protected.
But when it comes to the consequences the hero takes the one higher level of defeat like the follower normal would. So no other special rules required :-)
