For the record, this need not happen. The core rules say that the narrator *may* impose a penalty on a defeat (HQ p74) and FWIW I personally would generally not impose this kind of a penalty, barring a fumble or some story reason. That's why, when I glossed the rules for the Hero's Book, I specifically included an archery example which made it clear that when shooting at an enemy with no means of retaliation, the archer will generally be unharmed by any defeat (HB p22).
