Actually, it sounds like your question is about use of feats, not affinities -- since affinities are used (by initiates) only to augment, I would never let use of an affinity result in decapitation.
A complete victory is the same thing as -31 AP -- just achieved through means of a simple contest. Yes, I'd say the foe was decapitated if the player uses Decapitate Foe feat and achieves a complete victory.
I would not not allow decapitation to result in an extended contest if the foe were reduced to 0 AP. That would allow a feat to produce death on a marginal victory, instead of a Hurt.
