Well, she hasn't stopped running on the treetops when she cast the javelin, so I wouldn't make her roll again (at least not against the trees -- conceivably she might have to roll against the broos if they are attempting to get away from her).

In fact, I'd probably have her roll to get into position, then in succeeding rounds augment Throw Javelin with Run on Treetops.

No -- you can never use affinities as active abilities, only to augment. (FWIW, when I augment with an affinity, I usually mention the feat I'd use if I were a devotee, but I don't think this is a requirement.)

I'm not sure I agree with this (there is always a chance that magic doesn't work, due to the innate resistance of 14), but I also think this isn't a normal circumstance, it's in the heat of a battle.
