How does the D+x (increase in Difficulty) works exactly ?

For example, Irripi Ontor allows the feat Read Foreign Writings D+5.

What does that mean ? What is D in this case. How does that compare with another character having a skill of "Read Kralorelan" at 10W for
instance, if both try to understand an old Kralorelan Book. Who gets better chance to read it ?
