It would be from the Wounding of Korasting at the arrival of Yelm in Wonderhome. The devolution from Mistress Race to Dark Troll started with that event. Their fertility was decreased, their children borne stunted and pregnancy became a burden rather than a blessing.

Typical, it's all the fault of a Male with ideas above his station. (Yelm or Orlanth, take your pick)

So while it's probably not got anything to do with Grandfather Mortal dying, I'd say all Troll births are difficult for the Mothers (and really hard on the fathers :) ).
