Somewhere in the HeroQuest rules there is a paragraph or two about Spirits of Reprisal. Although they don't have Curse Apostate, the implication is that if you leave one religion for another, you get hit by a spirit of reprisal.

Exactly how this works is somewhat unclear, and I personally use the idea that how hard you are hit depends on how involved in the religion you are. (The higher your commitment, the harder you are hit.)
