Of course how to use it up to the individual(s). But, I suspect that in practice it occurs less than you (pl.) might have thought when writing the rules.

Let's say you reckoned that a PC getting into a serious contest has at least a 1 in 5 chance of ending up "dying" (across the many iterations of such contests). Seems like a fair enough ballpark figure to make contests a bit worrisome. Empirically, my suspicion is that it is more like 1 in 30, if that, but I've only my own experiences and some comments from Jane and Nic.

I suspect too that it is this feature which leads to the initial comments about the game which kicked this discussion off. It is possible to tweak those tables to make complete defeats more common. (Of course it can be house-ruled but that wasn't the original concern.)
