A radical suggestion: don't simulate it at all. What is the game to be about? If the game is about what happens as the PCs slip into insanity, that is the "setting" or "scenery" for the game, and needs no more rules than you needs rules for trees growing or mountains being solid. At the start of the game you could clearly say "This game will be 5 sessions long. Throughout the sessions, the characters gradually loose their sanity, becoming completely deranged at the end of the last session".

After all, it would suck if a PC went utterly insane in session 1 of the game, and would be weak if the PCs could survive the Unspeakable Horror with sanity intact.
