Most ancestors should be persuaded to enter a medicine bundle by means other than spirit combat (i.e. persuasion, bribery etc. which are mentioned in the rule). Only for evil ancestors would anybody think of using spirit combat to make a lasting fetish. Thus one would normally find a ancestor in a one-use fetish.

One could argue that an ancestor who has been overwhelmingly conviced of the rightness of the cause (i.e. complete victory), would hang around for multiple uses and at the end of the adventure, he would normally go away (unless perhaps a hero point is expended).
