Other examples - starting your deductions from a wrong premise (mistaking Doburdon for Durbadath, for example). Straining a muscle.  "Overdoing it" and sometimes just having bad timing.

Or you spill the stuff all over yourself instead of your blade and ye gods does it *itch*...
