Henchmen in D&D and AD&D from '78-'79 is the first reference I can find. Champions 1st ed came out in '81 or so (according to my very dog-eared copy).

Just went through the library and that's the earliest reference I coudl find. But I admit I did like how Ars Magica handled it -- NOT as followers but as a part of the troupe-style play (which is hard to do and requires a good group of players willing to put in a lot of energy...)
