The rules are on NB 50-52, called "Incarnation". Heroforming is the theistic way to incarnate, Channeling is the animist.
As Jeff pointed out, the story of Morden Defends the Camp is a primary source for what heroforming looks like (Both Morden and Big Man are heroforming)
