I use Adobe Illustrator and, if I have time :-), a bit of filters effects with Adobe Photoshop.

There are many fantasy carthographer applications, for the PC based people., they are good for speed, I suppose, but your maps will be similar to other maps. It is a standard , not necessary a bad thing

But I prefer illustrator
