I believe that the Hero plane is/are the various ages (mainly prehistorical)  of Glorantha and are entirely separate to the almost unchanging (*1) Philosophical worlds.

The Philosophical worlds are where the more powerful worshipped beings and their retinues etc. live. The Hero plane is where those same beings act/acted/have their worshippers re-enact/(none of the above)/(all of the above)/(something else) their myths out.

A major attribute of a Philospohical world is that it provides a connection to transcendance.
- The Spirit realm is pretty much unstructured. - The Gods world is semi-structured with levels but the barriers tend to be beings or apparently physical barriers such as doors or mountain ranges (*2).
- The Essence planes are highly structured into the levels Peter described, except that Hell is part of the Underworld.

The Underworld (like the Hero plane) is mixed (*3), though some sections are more strongly aligned with one of the Philosophical worlds (again the same as the Hero plane). But the trolls and elves and probably the other elder races (but not dragons and their kin) are associated with the Underworld rather than a Philosophical world.

The only way (at least until the Hero Wars are well underway) to go from one Philosophical world to another is via a mixed world, be it the mundane, an age of the Hero Plane or the Underworld.

*1 Orlanth is Dead gives an example of an empire wide, multi-year, high level and multi-conditional ritual that could temporarily change the gods world.

*2 I seem to remember from Thunder Rebels that there is a door in Orlanth's Hall that most cannot see and that has a 10w6 or 10w9 barrier to going through to trancendance. Or something.
