And I'd bet in many cases, most (or even all!) of the members of a particular subcult don't even know that they're worshipping Krarsht. They think it's just an assassin's cult, or a local hero, or a city god (in one of your nastier cities). You'd have to get deep into the mysteries before you found out who was really pulling the strings.
It might be Krarshtite SOP to set up a front cult when they're fairly successful in subverting a region. Once you've done that, you can let the region more or less run itself. After all, the locals think they're good monotheistic worshippers through the order of St. Maledictus the Patient. It just happens that all the good worship energy they send to *that* node ends up feeding the Maw--and if she ever needs more direct service, she's holding strings she can pull.
