Hm. Perfects are tricky. Right now I'm not sure how to deal with them, if at all.

One idea occurs, although I'm nowhere near experienced enough with HQ2 for this kind of systemic tinkering. It's very rough and likely vastly overpowered:

A player can declare a perfect during any contest where they use a named charm. The perfect negates all previous rolls and ends the contest with a complete victory for the player and a dying result for the opponent. No negative consequences of victory are experienced by the initiating character. A character assaulted by a perfect may counter with a perfect of their own if they can. Doing so results in a tie; the contest ends and no negative consequences are experienced by either side.

So what's the downside? Perfection cannot be used on two successive contests and victories gained are counted twice on the pass/fail cycle. No lingering advantages may be claimed from perfect victories.
