From the manual, outer patrols are your distant early warning line. They see raids coming and warn the clan, but generally don't participate in the defense, as they cannot get back in time.

Inner patrols are your first responders. They are the ones who call up the fyrd and fight the battles.

So, if you are missing raids, have lots of cattle going missing, up your outer patrols. If you are losing defensive battles, up your inner patrols.
