This did bother me more than a little which is why I chose Barntar as
one of the major Second Age Orlanthi Cults, specifically, the most
common non-warrior, non-weather-worker, non-Chieftain cult.
I think his worship is fairly common, particularly outside the rather
neo-conservative (ahem) Sartarites. Certainly he'd be more palatable
among the various Orlanthi who are not quite so overwhelmingly, well,
Orlanth-o-centric. Maniria or Hendrikiland or Tarsh, for example.
But YGMV. Mine does.
