I think he means as opposed to 'Three Otherworlds plus the Red Moon'. With the Moon being, in effect, a fourth Otherworld. It's not the way things work in my Glorantha (because My Glorantha Varies), but its the impression I get from the books as to how they're 'supposed' to work.
