I once was running a bog-standard fantasy RQIII game. The PCs were attacked by a mob of orcs. In the second round of combat, the orc leader fumbled and knocked his own helmet off. The next round, he fumbled and chopped off his own head (critical hit to random location).
It was glorious. The rest of the orcs became convinced that the PCs had cursed their toughest guy, and ran away immediately.
