...or it is "between sessions" and this is assumed to happen offstage (for free)...

The 5 "starting fetishes" are effectively items that you have "paid Hero Points for" (although, of course, you haven't) so generally, the GM should always let them be a part of your character. This would/should apply to 1 use fetishes that you pick up during play - If you spend a HP to add them to your Character Sheet then they become a "part of" your character - if you don't then they really are "1 use" (maybe not even that - if you don't use them at all then they may be deemed to have been used "off stage")
