A Theist with no access to Holy Day ceremonies will soon be screwed (the gods don't look favorably on non-attendance). A Monotheist with no access to a liturgist is even more screwed. Your point being?
The system assumes that you are playing a group with *some* access to normal society. Normal Animist society includes people who can/will "refill" your fetishes for you. Now, if you take your typical adventuring party of a Praxian, Great troll, Orlanthi, Sundomer and Duck to the Far West, well then, yes, you'll be screwed.
Hero Quest will address these problems, but even then; if you operate too far outside of the norm you'd still better have some extraordinary magical support, because operating on the edge is *dangerous*.
