While "the party" succeeded, the two were killed individually during combats (Hazeel during the "Attack of the Great Wolf" station (pg 202) and Mr. Puma fighting the Isfalt and the Crab People).

In Game terms? The took part in Extended Contests, and at the end of those contests there were at -31 or below, and Kathy decided that they were dead. Note that Jane tried to heal Mr Puma during the contest but was defeated herself when "her parts fell off" (Jane was probably brought to "Injured" rather than "Dying", but either way...)

Just because one side wins an Extended Contest doesn't mean that everyone on that side is unharmed - anyone who ends the contest with Positive AP is (relatively) unharmed, but everyone else suffers wounds consistant with their Negative AP (Contest Consequences Chart, pg 74). Even the "winners" could be suffering the effects of the "Trading AP for Penalties" rule (pg 189), or the effects of having been knocked out of the fight but recovered through Final Action (pg 74) (remember that you still suffer the negative effects of your wound if you come back via FA, so if you successfully FA back from "Injured", you've got a -50% penalty for the rest of the fight, and for a month or more afterwards).
