IMO, the rules give way to any option.
If the Narrator thinks that a thing is necessary for the scenario, it is so.
It's like her spending the HPs to cement the "exception to the rules" feature which is key to HeroquestTM.
Rules in HQ serve the purpose of the story, and this is not just chatter.

If at the end of the scenario the heroes spend HPs to cement their fetishes then *they* have to justify it (by the background). If they don't want to, don't feel so or merely spend their HPs elsewhere, then wave your narrating hands and say "oooh, it's gone".

Mistery is mistery.
