I think what may be needed here is narrator advice that the type of damage suffered by followers will depend on their role and how they participated in the contest, it need not be the same as that of the lead character.

In this case the healer's magic clearly failed or else the warrior would not have been too injured to continue. Or perhaps the failure of her healing magic has caused her to doubt her abilities and her goddess. Whatever the cause of this the effect is that she now suffers a penalty to her healing as an outcome of the contest. Physically she is fine but magically she is injured. So she can drag the hero out of the combat but he is going to have a long slow time healing unless he can find someone else to help out with magical healing - or with helping his healer regain her magic.
