Depends on context.
In some cases use the default resistance (e.g., lifting an opponent). In others, use an appropriate ability (e.g., close combat dodge, agility or whatever to resist). If no suitable ability exists I would probably argue that 6 should be used.

Yes. In practice this is quite useful, but not unbalancing (as it is quite rare). ISTR this a "narrator decides" outcome. I usually rule "fight to a standstill" or similar.
