The only example that I know about is Harshaltar the Terrible breaking a geas in combat with the Emperor and being killed. But he might probably was a disciple for all we know. For initiates and devotees, for all I know, they lose the associated gift, an agent of reprisal visits them then and at any time they break the geas again (and possibly another geas that they've acquired as a curse).
