Canonically, it hasn't happened yet (if, indeed, it ever will). It was the basis of a freeform game held at Convulsion back in the day, but this game was set a few years after the date that most supplements are set.

Yes; it was the Rokari that were instrumental in organising it. They hoped to get everyone else to agree that Rokarism was the One True Path - from which perspective, they obviously weren't very successful.
