The "little break" was made by Jorbal Rhino Kahn (sic) when he summoned the long-dead Great Rhino to push down the Wall. It occured after Jaldon chewed down the walls and is described as the Second Break.

Gerak Kag and his trusty companions entered Pavis by leaping atop the walls rather than making any breaks. Hence the Troll Break is named because it's right next to where the trolls are.
