All of the above, effectively. Utuma and final draconic transformation are, even outside Kralorela, often equated; in KraLor, this is also explictly linked to progress to the Summer Land Heaven (Vithela), both for the Emperor himself -- he is, after all, a cosmicly immense being selflessly constraining himself to the human scale for the benefit of his subjects -- and for those already demised wishing to accomplish their final translocation from the SLH to a state of Liberated Bliss. (Think Mahayanan bodhisattvas in general, and Pure Land buddhism in particular.)

Let's just say that not always is the wisdom of the dragons' cosmic plan always readily perceptible to the unsophisticated eye (e.g., God Learners invade country, Emperor commits suicide).

I believe much is true in Kralorela, though of course there have only been two 'usurpers'. (Unless you want to invoke the cynical explanation of 'history gets written by the (successful) usurpers', so at any rate such as there have been have rarely been subsequently 'reversed'.)
