If you want the cave troll to be scarier during the contest, you could allow the cave troll to perform an Assist on itself -- this is naturally limiting since subsequent attempts are harder.
I think in a few cases (like Chaotic Feature regeneration) I've decided ahead of time on a "removal budget" so that I'd remove (say) 2 AP markers over the course of a contest. Even if the budget is 1, I think it gives players a healthy respect for the monster when they see (in game terms) it getting better.
