I got the distinct impression that this was a film created by, and for, people who already knew exactly what to expect... despite the cuts and changes to the book, the vast majority of the core audience (the ones who went to the first showings, then dragged their friends along and told everyone else, and started the ball rolling) would already know about the Balrog anyway. It's only since the film proved such an incredible success that large numbers of people who never heard of Tolkein have seen it.
The phrase "They've got a cave troll!" will enter RPG history, if it hasn't already...
