This is often true of polls - even when the issue may seem clear cut... should Jeff's Duck poll, for instance have said "Ducks,Yes? No? or None of the Above?"

Which is partly the point, a chance to discuss how these issues may affect, or have affected your game(s).

How a new character is introduced will depend on who the character is and where the rest of the PC's are when he appears. If he is another member of their clan then the character may "always have been there" -  it's just that we've never noticed them before. If they are an Uzdo Zorak Zorani from the Shadow Plateau joining your East Islands campaign, then obviously the Player and GM will need to work out just how they get there...

When a player leaves a game then what happens will probably depend on the current campaign situation, why the player is leaving, and whether they plan on ever coming back (and even, whether you know they are leaving in time to plan anything). Some players will welcome the chance to die a heroic death at the climax of (their time in) the campaign - on the other hand, it's a bit unfair to kill someone off if they are off to college, and will be returning to your game at Christmas.

As an alternative they could marry, settle down, or just go off on a long journey (physical or spiritual). If the player does then reappear  either permanently, or just for the occasional session the "old" character can be brought out of retirementto aid their old colleagues.

One option HeroWars does offer is that PC's can come from, or become, followers (OK so the skills may be a little bit out for "the rules", but it's a way with dealing with the loss of a Player at a point where it is impractical to lose the character)
