All sorts of delays can creep in to the printing process. Printers can, for example, print the first 1000 copies in a batch run and bind them with the fourth signature completely inverted (so that pages 49 through 64 appear backwards and upside down). It has been known to happen.
If a significant problem occurs in a print run, and large numbers of books (or even bare printed pages) need re-doing then printers don't typically immediately rerun a job; they slot the rerun into the next available slot in their schedule. In this way, the printer pisses off only one customer, instead of *all* their customers. This can mean significant delays for individual jobs that have problems, though.
