I'll just repeat the advice from Dan Greenberg, one of the best GMs I've ever run under:

If you want to scare the hell out of players, mess with the bodies of their characters.

strange growths under the skin, vomiting substances they never ingested, the feeling of something moving in their abdominal cavity, etc.

This could all be the result of some magician with a "fear" spell of some kind.

The coolest example of messing with the PC's bodies I know of was when a psychic PC in one of Dan's games tried to locate a mysteriously missing character. Unknown to the PC, this character had drowned a short time before. When the psychic entered a trance and "passed out" to reach the missing person, her success at finding the person was marked by gallons of sea water flowing out of the psychic's mouth.
