We use CafePress for shirts and mugs with KoDP artwork. I like them for that, though they have a pretty long delay for payment (because items are returnable).
As it happens, I'm investigating CD-on-demand from eSellerate (which we use for e-commerce for our Mac OS X titles). That might work, though I don't know any way they can create cross-platform CDs like the original. And they don't print on the CD like CafePress.
Price for either option should end up the same, should we go this route.
