Unfortunately this is down to Lulu.com, and the way their service handles the files. What you are buying when you get the PDF, is the same print ready pdf that Lulu uses to print off the print version and the cover is a separate file, which is a single file including spine, which they don't bundle together with the download.
I used to do an ebook version of the magazine, which contained covers and was fully bookmarked, but it was a smaller file size and therefore not print quality. I received some feed back from people who wanted the pdf download to be print quality, so I made the decision to stop doing the ebook version and simply make the same pdf that lulu uses to print off the magazine. While unfortunately this file does not include the covers (or bookmarks), I thought it easier to have only one pdf version of the magazine when people came to order the magazine from the Lulu storefront.
However if there is demand for it, I am willing to reinstate the ebook version.
So I've started a poll on the d101games forum to see if people want the e-book version
