I think we're talking about two things that are different. If you just enter the _hero_ plane from anywhere in the wastes, it is really easy to find yourself in one of the bad places, and those connect pretty quickly to the devastation of the weeping times. But, if you have already gotten to the specifically Praxian part of the _spirit_ plane, it is very difficult to get there. You essentially need to find a way to leave it, and there are limited connections from it to anywhere.
I was assuming Mike was talking about the punitive sort of hell that some religions talk about, so was referring to the latter. I suspect that your assumption was that he wanted to know about places in the otherworld more generally where bad things come from, so were talking about the former.
